All visits are by appointment only. You will need to arrive 20 minutes early for the first visit and fill out the forms. Appointments will be 60 minutes. Bellevue’s East Asian Wellbeing acupuncture clinic is open Tuesday through Saturday from 9:00 am – 6:00 pm.

Dr. Miao is a preferred provider for the following insurance companies:

• Kaiser Permanente Access PPO
• Regence
• BlueCross and BlueShield
• Aetna
• Meritain

Dr. Miao also accepts auto accident claims PIP (Personal Injury Protection). No referral is needed for auto accident injuries.

There is a 1-800 phone number on the back of your insurance card. Please call your insurance company to find out if your plan includes acupuncture benefits.

If you have a different insurance company it is possible that treatments with Dr. Miao will still be covered as an out-of -network provider. Please check with your insurance company.

Treatment fees vary based on the type of service provided, so please call to receive an estimate for the cost of your office visit.

Payment is expected at time of service. Dr. Miao accepts check, cash and credit cards (Visa, Mastercard, American Express, Discover).

If you prefer to have us bill your insurance company, you are still responsible for any co-pay, deductibles and the cost of herbal supplements at time of service.

If you are late, please call us as soon as possible. We will try to accommodate you to the best of our ability, but your treatment session may be shortened to prevent running over into the next patient’s scheduled time.

If you need to cancel your appointment, please give 24 hours notice, otherwise a $50 fee will be charged for missed appointments or cancellations with less than 24 hours notice.
